رپورٹ میں ناصرف اْس واقعہ کی بنیادی وجوہات بیان کی گئی ہیں بلکہ اس حوالے سے ذمہ داران کی نشاندہی اور آئندہ ایسے واقعات سے بچنے کیلئے تجاویز بھی دی گئی ہیں۔ اجلاس کو بتایا گیا کہ 23 جنوری 2023 کو صبح ساڑھے سات بجے سے بجلی کا بریک ڈاؤن پاور پلانٹس کے ٹرِپ ہونے سے صرف چار منٹ میں پورے ملک میں پھیلا۔ اس کی بنیادی وجہ انسانی غفلت تھی جو کہ رات کو بند کیے گئے اے سی (AC) سرکٹس کو صبح کے وقت سوئچ آن نہ کرنا تھی. جونہی ملک کے شمال میں بجلی کی طلب کو پورا کرنے کے لیے جنوبی علاقے میں وِنڈ انرجی سے پیدا کی جانے والی بجلی سسٹم میں شامل کی گئی تو اے سی (AC) لائن کی فریکونسی غیر معمولی طور پر بڑھ گئی۔ اس فریکونسی میں یکدم اضافے سے مختلف پاور پلانٹس خودکار سیکیورٹی سسٹم کے تحت ٹرِپ کرگئے۔
پاور پلانٹس کو بلیک اسٹارٹ سسٹم کے ذریعے رِی اسٹارٹ (Restart) ہونا تھا لیکن اس نظام کی ناکامی کی وجہ سے کئی علاقوں میں بجلی کی بحالی میں خاصی تاخیر ہوئی۔مزید برآں اجلاس کو آگاہ کیا گیا کہ کوٹ اَدو پاور کمپنی جس کے پاس بلیک اسٹارٹ کی سہولت موجود ہے لیکن اس پاور پلانٹ کے معاہدے کی معیاد ختم ہوچکی ہے۔اجلاس کے شرکاء سے گفتگو کرتے ہوئے وزیراعظم نے کہا کہ قوم منتظر ہے کہ اس بڑے واقعے کے ذمہ داران کو اْن کی اس سنگین لاپرواہی پر قرار واقعی سزا دی جائے۔ اْنہوں نے ہدایت کی کہ پاور ڈویژن اس مجرمانہ غفلت کے مرتکب اہلکاروں کے خلاف فوراً محکمانہ کارروائی کرے۔ وزیراعظم نے استفسار کیا کہ کیپکو کے پاور پرچیز کے معاہدے کی معیاد اگر ختم ہوچکی تھی تو اس معاہد ے کی تجدید کے لیے ضروری کارروائی میں تاخیر کیوں کی گئی۔
